Breast augmentation is one of the more common procedures. In fact with 300,000 procedures per year in the United States, it is the most common form of cosmetic plastic surgery in the country.
Breast augmentation is the name used when the operation is for cosmetic reasons. If the implants are needed because breast tissue has been removed because of cancer (mastectomy) the operation is called breast reconstruction. Some medical conditions can also stop the breasts developing normally, and in those cases breast reconstruction may also be necessary.
The implants can be either saline filed, or silicone-gel filled. Saline filled implants sometimes suffer from the disadvantage that rippling or wrinkling in the implant itself may be visually noticeable. This can be a particular problem if there is not much breast tissue to cover the implant.
On the other hand saline filled implants are implanted empty into the breast, and the saline liquid is added afterwards. This means that the incision can be considerably shorter. Incisions around the areola may be possible here. The areola is the area of differently pigmented skin around the nipples. Depending on the actual color of the areola a small scar here may be less noticeable than a larger scar below the breast, in the inframmamary fold.
To insert a silicone gel filled implant an inframammary implant is more likely. Silicone gel implants can suffer from silent rupture, and the patient requires frequent MRI scans after the operation. For this reason breast augmentation with silicone gel is restricted to women aged 22 years and upwards. There are no similar restriction for breast reconstruction.
In most US augmentation procedures the implant is placed in the subpectoral position. The implant is under the pectoralis major muscle after releasing the lower attachments to the muscle. This ensures good coverage for the implant, hiding any wrinkling/rippling. The slightly different submuscular implant pocket tends to be used for breast reconstruction.
