This means that the pain is mostly psychological rather than physical in nature. For example, if you slipped in the bathroom three years ago, your subconscious may still continue telling you that there is pain in the coccyx area when in fact, the actual injury should have completely healed by that time.

Another reason could be that some people suffer from coccyx pain due mainly to a lack of oxygen in the area. In this case, the root cause may be a bit difficult to pinpoint, which makes it also difficult to identify the appropriate treatment method.
