Once the diopter is set this will more or less stay in position, so accidental contact will not affect a shooter’s capacity to rack focus.

Special lenses for film cameras are usually mounted on a fixed / integrated ring gear , while for other lenses, for example in the field of video DSLRs are clip-on sprockets. A gear on the focus system intervenes in these sprocket. The gear is in turn (possibly via another toothed wheels) connected to the rotating ring. To adjust the focus puller for different lens diameters, the gear is mounted on a pivotable arm.
