What is Diabetic nephropathy (DN) and Captopril drug?
Diabetic nephropathy (DN) is typically defined by macroalbuminuria—that is, a urinary albumin excretion of more than 300 mg in a 24-hour collection—or macroalbuminuria and abnormal renal function as represented by an abnormality in serum creatinine, calculated creatinine clearance, or glomerular filtration rate (GFR). Clinically, diabetic nephropathy is characterized by a progressive increase in proteinuria and decline in GFR, hypertension, and a high risk of cardiovascular morbidity and mortality.

Mode of action of Captopril. Blocks the angiotensin-converting enzyme, resulting in the formation of angiotensin II inhibits and eliminates its narrowing effect on the arterial and venous vessels. Reduces the total peripheral vascular resistance, decreases afterload, reduces blood pressure. Decreases preload, reduces the pressure in the right atrium and the pulmonary circulation. Reduces the formation of aldosterone in the adrenal glands.
Indications. Hypertension (including, and renovascular), chronic heart failure.
Side effects. The drug is generally well tolerated.
Side effects are generally mild and transient dizziness, headache, fatigue, nausea, loss of appetite. There are – orthostatic hypotension, fatigue, skin rash allergic genesis. Rarely – tachycardia, abdominal pain, diarrhea, angioedema, worsening renal function, proteinuria, hyperkalemia, elevated liver enzymes, an increased concentration of bilirubin in the blood neutropenia. It is very rare in patients with autoimmune diseases – agranulocytosis.
Contraindications. Severe impairment of renal function, azotemia, hyperkalemia, bilateral renal artery stenosis or stenosis of the artery of a solitary kidney with progressive azotemia, a condition after kidney transplantation, primary hyperaldosteronism, aortic stenosis and similar obstacles to the outflow of blood, hereditary angioedema, pregnancy, lactation, infancy, hypersensitivity to captopril and other inhibitors of converting enzyme.
